Please type the date and location of the Incident report you are requesting. If you are requesting a Emergency Medical Report, we must have a signed release from the patient prior to releasing this info. There is a fee of $10 for each report. You will receive an email when your report is ready to be picked up. Please include your name address phone number & email with your request.
Request for a 21E Lt Keeman:
Please include the property address, and last known Business name if available. When your report is ready you will receive an email. Please include the name of your business, your name, address telephone number, and email address. There is a fee of $25 for this report.
